RESUMO

As the environmental foundation for tumor survival,the tumor microenvironment is characterized by immunosuppression, ischemia and hypoxia, metabolic reprogramming, and inflammatory infiltration, providing conditions for tumor cell colonization, malignant transformation and migration. In traditional Chinese medicine (TCM), yang qi is considered to be closely related to the onset of tumors. Based on the TCM theory of “where there is mass hardness, there must be latent yang”, this paper suggests that the state of “latent yang” formed by abnormal yang energy is highly similar to the tumor microenvironment. The loss of protection of latent yang leads to the weakening of positive immunity, invasion of disease and then accumulation of tumors. The constraint of latent yang leads to microcirculation disorders, blocked vessels and collaterals, and finally qi and blood blockage. The turbidity of latent yang leads to abnormal metabolic products gathering, pathogenic turbidity accumulation, and tumor formulation through contention and binding between them. The transformation of fire by latent yang mediates “inflammation-cancer” transformation, and leads to malignant transformation of cancerous tumors. The transformation of latent yang into toxin leads to premeditated pre-metastatic niches, healthy qi deficiency and toxin accumulation, and cancerous toxins flow. Based on the theory of “latent yang” and modern researches, this paper summarized the method of “warming yang and invigorating qi to consolidate the root, promoting yang and moving qi to eliminate stasis, strengthening yang capacity of transforming qi to remove the disadvantages, diffusing yang and opening up constraint to disperse fire, and reinforcing healthy qi and clearing the source to prevent toxins” in the intervention of the tumor microenvironment, so as to provide ideas for TCM to reshape the tumor microenvironment and regulate the tumor process.

Objective:To evaluate the value of systematic education for primiparas undergoing epidural labor analgesia.Methods:A total of 240 primiparas who were suitable and voluntarily requested epidural labor analgesia, aged 20-35 yr, at 37-41 + 6 weeks gestation, with a singleton fetus, in vertex presentation without fetal abnormality, were selected.According to whether the primiparas and their accompanying family members had received systematic education on epidural labor analgesia during pregnancy, the primiparas were divided into 2 groups ( n=120 each): education group and control group.When the uterine contraction was regular and the diameter of the uterine orifice reached 2 cm or more, epidural labor analgesia was carried out.Before the begining of epidural puncture, the degree of anxiety of pregnant women was evaluated using the self-rating anxiety scale.The time of communication before analgesia, time of placing body position, and satisfaction of puerperae and their family members with labor analgesia were observed and recorded. Results:Compared with control group, the self-rating anxiety scale score was significantly decreased, the time of communication before analgesia and time of placing body position were shortened, and the rate of satisfaction of puerperae and their family members with labor analgesia was increased in education group ( P<0.05). Conclusions:Systematic education is helpful in relieving the anxiety of primiparas during epidural labor analgesia, increasing the efficiency of analgesia implementation and improving the delivery experience of puerperae.

OBJECTIVE@#To analyze the (CGG)n repeats of FMR1 gene among patients with unexplained mental retardation.@*METHODS@#For 201 patients with unexplained mental retardation, the (CGG)n repeats of the FMR1 gene were analyzed by PCR and FragilEase@*RESULTS@#For the 201 patients with unexplained mental retardation, 15 were identified with full mutations of the FMR1 gene. The prevalence of fragile X syndrome (FXS) in patients with unexplained mental retardation was determined as 7.5% (15/201). Prenatal diagnosis was provided for 6 pregnant women with pre- or full mutations. Analysis revealed that women with mental retardation and full FMR1 mutations exhibited a skewed XCI pattern with primary expression of the X chromosome carrying the mutant allele.@*CONCLUSION@#FXS has a high incidence among patients with unexplained mental retardation. Analysis of FMR1 gene (CGG)n repeats in patients with unexplained mental retardation can facilitate genetic counseling and prenatal diagnosis for their families. FMR1 gene (CGG)n repeats screening should be recommended for patients with unexplained mental retardation.

RESUMO

The widespread application of next generation sequencing (NGS) in clinical settings has enabled testing, diagnosis, treatment and prevention of genetic diseases. However, many issues have arisen in the meanwhile. One of the most pressing issues is the lack of standards for reporting genetic test results across different service providers. The First Forum on Standards and Specifications for Clinical Genetic Testing was held to address the issue in Shenzhen, China, on October 28, 2017. Participants, including geneticists, clinicians, and representatives of genetic testing service providers, discussed problems of clinical genetic testing services across in China and shared opinions on principles, challenges, and standards for reporting clinical genetic test results. Here we summarize expert opinions presented at the seminar and report the consensus, which will serve as a basis for the development of standards and guidelines for reporting of clinical genetic testing results, in order to promote the standardization and regulation of genetic testing services in China.

Objective To study the success experience of native arteriovenous fistula (AVF) operation,and provide a new reference range of blood vessel diameter for evaluating the success rate of AVF operation before surgery.Methods To proceed AVF operation to patients with 5 phase of chronic kidney disease,before surgery,patients should receive the vascular ultrasound and cardiac ultrasound to record the diameter of radio artery and cephalic vein along with the radio artery blood flow.Patients were divided into two groups:observation group(cephalic vein diameter< 2 mm)and control group(cephalic vein diameter > 2.0 mm). All those indexes examined before were re-examined 1 month after surgery. Except vessel diameter, the AVF blood flow velocity was also recorded. The range of the mature period was measured, along with the blood flow of each AVF 1 month after its initial use. The blood vessel diameter before surgery,the mature period of AVF and the blood flow on the machine of dialysis 1 month after the initial use were measured to evaluate the operation results of each group. Results In the observation group, 23 of the 25 patients showed the optimistic results. Unfortunately, 1 of the 2 pessimistic results showed the absence of vascular murmur,and the other one died of natural death before its initial use.The mature period of AVF for hemodialysis was(47.29 ± 2.45)d,significantly longer than that of control group (36.00 ± 1.29) d, and there was statistical difference (P<0.01). The blood flow of observation group showed no significant difference compared to that of control group (P > 0.05). Conclusions The patients with cephalic vein diameter<2.0 mm before operation needs a long mature period,but the blood flow after dialysis is not affected,and a good success rate is obtained.

Objective To investigate the prevalence of hypertension in high temperature,noise operators and related factors.Methods A total of 1 263 workers from a steel enterprise were selected,among them,583 operators contacting with high temperature and noise served as the group A,267 operators only contacting with noise as the group B,249 operators only contacting with high temperature as the group C and 164 operators without contacting with high temperature and noise as group D.The prevalence of hypertension and related factors were analyzed.Results In the analysis of hypertension prevalence rate,the group D (12.8%) < group C (25.7%)< group B (34.6%)<group A(43.9%).In the analysis of blood pressure(systolic pressure/diastolic pressure),the group D[(115.8± 11.5)/(73.6±5.4) mm Hg]< group C[(124.1±10.7)/(81.9±7.3) mm Hg]< group B[(132.9±11.3)/(86.7±5.2) rnm Hg]< group A [(143.7 ± 12.8)/(92.4 ± 6.5) mm Hg].The onset age of hypertension in the group A was earlier than that in the group B,C,D (P<0.05).The lower educational level,the more working years,the bigger body mass index(BMI),themore smoking and drinking were,the higher the hypertension prevalence rate was.Hypertension had significantly negative correlation with the educational level and significantly positive correlation with age,working years,BMI,smoking and drinking (P< 0.05).Conclusion The prevalence of hypertension in high temperature and noise operators is related to personal constitution and living habits.

<p><b>OBJECTIVE</b>To analyze the karyotypes and SRD5A2 gene mutations in 25 patients with sporadic or familial hypospadias.</p><p><b>METHODS</b>The patients included 10 adults and 15 children, whose chromosomes were analyzed by G-banded karyotyping, and the SRD5A2 genes were sequenced.</p><p><b>RESULTS</b>Two patients were found to have an abnormal karyotype, while eight have carried compound heterozygous mutations of the SRD5A2 gene, which included 5 genotypes formed by 6 types of mutations, i.e., p.G203S/p.R227Q, p.R227Q/p.R246Q, p.Q6X/p.Q71X, p.L20P/p.G203S, and p.Q71X/p.R227Q. Mutations of the SRD5A2 gene were present in 32% (8/25) of all patients, 35% (8/23) in those with a normal karyotype, and 44.4% (8/18) in those with proximal type hypospadia. Bioinformatic analysis, literature review and pedigree analysis confirmed that all such mutations are pathogenic.</p><p><b>CONCLUSION</b>Chromosomal anomalies and mutations of the SRD5A2 gene are the main cause of hypospadias. Sequencing of the SRD5A2 gene may explain the etiology of nearly half of the patients with proximal type of hypospadas but a normal karyotype, which can facilitate genetic consulting.</p>

Objective To investigate the specific growth factor tumor (TSGF) in the treatment of chemoembolization (transcatheter) in the treatment of primary hepatic carcinoma (HCC) with the change of the level of arterial and transcatheter arterial chemoembolization (TACE),and the evaluation of the value in the treatment.Methods From January 2012 to December 2014 in Inner Mongolia Medical University affiliated Hospital,75 patients with primary liver cancer were treated by TACE,and all patients were treated with TACE.Then the changes of liver function,blood routine,alpha-fetoprotein (AFP),and TSGF levels were monitored at the preoperative,postoperative 1 week,1 month,and 3 months.The curative effect was evaluated.Results At 1 week after operation,aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were significantly higher than those before operation,but no differences were compared to the preoperative 1 month after operation.There was no significant difference in the level of WBC before and after operation.At 1 week after operation,the averaged AFP and TSGF water were significantly increased,but the 1 week after operation was significantly decreased.At 1 month after operation,there was no difference in the levels of TSGF and AFP between complete remission + partial remission + stable disease (CR + PR + SD) and progress disease (PD),and the levels of AFP and CR + PR + SD in PD patients were significantly lower than those in patients with TSGF at 3 months after operation.The levels of TSGF and AFP were lower in PD patients after 3 months,and there was no difference between TSGF and AFP levels at the different time after treatment in patients with CR + PR + SD.There was no significant difference in AFP levels before and after AFP treatment,and the average water level of TSGF before treatment was significantly higher than that of TACE before and after treatment.The difference was statistically significant (P < 0.05).Conclusions TSGF level in patients with primary liver cancer patients with TACE treatment increased after the first rise,especially in the treatment of 1 month after the deterioration of the situation and the tumor has a significant correlation.In the evaluation of curative effect,it has a high value.Combined with AFP,CT examination can increase the long-term efficacy of TACE treatment of primary liver cancer.

<p><b>OBJECTIVE</b>To detect SCN4A gene mutation in a pedigree with paramyotonia congenita in order to facilitate genetic counseling and assisted reproduction.</p><p><b>METHODS</b>Clinical data of the family was collected. DNA was extracted from peripheral blood samples. Potential mutation of the SCN4A gene was screened using PCR-Sanger sequencing. Potential mutation was detected in 3 affected relatives, 4 unaffected relatives and 100 unrelated healthy controls. Bioinformatics software was used to predict the effect of mutation on the protein function and conservation of the sequence at the mutation site across various species.</p><p><b>RESULTS</b>A novel missense mutation c.4427T>C (p.Met1476Thr) was detected in the exon 24 of the SCN4A gene in the proband and other 3 affected relatives, but not in 4 unaffected relatives and 100 unrelated controls. Bioinformatic analysis indicated that the codon is highly conserved across various species, and that the mutation has caused damage to the structure and function of SCN4A protein.</p><p><b>CONCLUSION</b>The c.4427 T>C (p.Met1476Thr) mutation of the SCN4A gene may contribute to the paramyotonia congenita. Detection of SCN4A gene mutation is an effective method for the diagnosis of paramyotonic congenita.</p>

Objective To investigate the implementation and application of rotational DSA dual volume technology in displaying the intracranial artery stent.Methods Firstly,the “stent opacity”single volume technology was used to display 5 types of stent which were released in 7 cases of pure intracranial artery stenting and 12 cases of stent-assisted aneurysm embolization.Then we applied “vessel translucent”and “vessel opacity”single volume technology to display corresponding vessel segments.Finally,by coalescing the single volume imaging of stent and vessel,the “vessel opacity + stent opacity”dual volume image and “vessel translucent+ stent opacity”dual volume image were performed.We compared the effects of two kinds of dual volume imaging and assessed the imaging characteristics and influencing factors of each method.Results (1)On the “stent opacity”single volume display mode,in 7 cases of pure intracranial artery stenting,the display effect of two Pipeline stents and three Apollo stents belongs to level 1,the display effect of two Enterprise stents belongs to level 2;In 12 cases of stent-assisted aneurysm embolization,in the stent segments which weren’t influenced by artifacts,the display effect of two LEVIS stents belongs to level 1,the display effects of 5 Enterprise stents and 2 Solitaire AB stents belongs to level 2;While in the stent segments which were influenced by the artifact of spring coil and bone,the display effect significantly reduced, three pieces of Enterprise stents were even unable to identify.(2)On the “vessel opacity”single volume display mode,1 9 cases of vessel segment at the stent completed opacity display.The surface structure of vessel was very clear,but the structure under the surface was completely being covered.On the “vessel translucent”single volume display mode,the surface structure of 19 cases of vessel segment at the stent displayed clear at the tangential position.The vessel lumen also presented transparent state.(3)On the “stent opacity+vessels opacity”double volume display mode,the start-stop location and the adherence of stents in blood vessels in 1 6 cases were displayed clearly. When using the “stent opacity+ vascular opacity”double volume imaging,except 3 cases which used ball expansion Apollo stents could display comprehensive relationship between the blood vessels,in the other 13 cases,the relationship between stent and vascular could not be fully displayed.Conclusion On the condition that the single volume display technology shows stent well,the “stent opaque+vessels translucent”double volume display technology can well present the stent condition in the vessels.

Objective To observe the effect and analyze the advantage of Qilian Fuzheng capsule in maintenance treatment of advanced non-small cell lung cancer. Methods The advanced non-small cell lung cancer patients were randomly divided into observation group and control group, 32 cases in each group. The control group was given the best supportive care, and the observation group was given Qilian Fuzheng capsule 4 capsules three times daily additionally. The tumor control rate, quality of life, PFS and OS of the two groups were compared by follow-up. Results Qilian Fuzheng capsule maintenance treatment of advanced non-small cell lung cancer improved the quality of life. PFS of observation group and control group was 5.4 months and 4.1 months, respectively (P0.05). The tumor control rate of observation group was 77.42% (24/31), control group was 60.00% (18/30), with no significant difference (P>0.05). Conclusion Qilian Fuzheng capsule is a feasible application in maintenance treatment of advanced non-small cell lung cancer.

<p><b>OBJECTIVE</b>To investigate the influences of recasting on the mechanical properties of Au-Pt ceramic alloy.</p><p><b>METHODS</b>Au-Pt ceramic alloy samples were prepared and recast for 3 times without adding any new Au-Pt ceramic alloy. The tensile strength, 0.2% yield strength, percentage of elongation, flexural strength, flexural modulus and Vickers hardness of each specimen were measured.</p><p><b>RESULTS</b>Being cast for different times, the Au-Pt ceramic alloy showed no significant differences on their tensile strength, 0.2% yield strength, percentage of elongation, flexural strength or Vickers hardness. The flexural modulus of the Au-Pt alloys being cast for 2 or 3 times was significantly higher than that of the alloys being cast for 1 time (P < 0.05). CONCLUSION Au-Pt ceramic alloy can be recast for 3 times at least, without any decrease in the mechanical properties.</p>

Objective To discuss the clinical characteristics and the relationship with the survival of smoking lung cancer patients.Methods The clinical data of 368 cases were retrospectively analyzed by comparing the differences of the age,gender,histological type,clinical stage,treatment and tradition Chinese medicine typing,and their correlation with the survival was analyzed.Results Among the 368 patients,the ones aged between 50 to 69 accounted for 69.84% ; the proportion between the male ones and the female ones was 7.98 ∶ 1 ; the proportion of the squamous cell carcinoma was as high as 52.45% ; the proportion of the patients with small cell lung cancer in extensive stage was significantly higher than that of the patients in the limited stage(75.38% ∶ 24.62%) ; the numbers of the patients with non-small cell lung cancer in stage Ⅲ and Ⅳ was significantly higher than that in the stage Ⅰ and Ⅱ (82.77% ∶ 17.23%) ; the ones with non-surgical treatment accounted for 76.36%,and the proportion of ones with surgery treatment was 23.64% ; most of the patients were treated by both Chinese medicine and chemotherapy(41.04%) ; the main types of all patients were qi-yin deficiency syndrome and toxic heat flourishing syndrome,accounting for 36.96% and 29.28% respectively.The age (F =10.356,P =0.000),clinical stage (F =11.289,P =0.000),treatment methods (t =2.624,P =0.021),smoking index(F =3.681,P =0.021) were the key factors effecting the survival (P < 0.05),and the gender,histological type,tradition Chinese medicine type had no significant effect(P > 0.05).Conclusion The clinical characteristics of the cases are 50-69 aged,male,squamous cell carcinoma,advanced stage,treatment without surgery.The survival of smoking lung cancer patients is effected by the age,clinical stage,treatment methods and smoking index.

ObjectiveTo discuss the effects of adsorption(AP) combined with continuous venovenous hemofiltration(CVVH) on the serum inflammatory mediators levels in systemic inflammatory response syndrome (SIRS) patients.Methods Sixty-three SIRS patients were divided into treatment group (31 cases,AP combined with CVVH ) and control group (32 cases,CVVH ) by random digits table method.The changes of the serum tumor necrosis factor-alpha (TNF-α ),C-reactive protein (CRP),interleukin (IL)-1,IL-6 and IL-10 before and after treatment were compared in two groups.ResultsThere was no significant difference in the serum TNF-α,CRP,IL-1,IL-6,IL-10 before treatment between two groups (P > 0.05 ).The serum TNF- α,CRP,IL- 1,IL-6,IL- 10 decreased after 2,26 and 50 h treatment compared with those before treatment in treatment group[after 2 h treatment:(226.4 ± 27.6) ng/L,(70.4 ± 22.1 ) mg/L,(30.1 ±2.9) ng/L,(227.5 ± 13.2) ng/L,(40.0 ±5.2) ng/L; after 26 h treatment:(165.3 ±24.5) ng/L,(58.2 ±25.1) mg/L,(18.2 ±2.7) ng/L,(82.4 ±7.2) ng/L,(26.2 ±4.3) ng/L; after 50 h treatment:( 120.6 ± 19.2) ng/L,(46.2 ± 24.6) mg/L,( 12.4 ± 2.3 ) ng/L,(38.1 ± 4.4 ) ng/L,( 15.2 ± 2.1 ) ng/L; before treatment:(350.8 ± 40.2) ng/L,( 126.4 ± 34.6) mg/L,(38.2 ± 3.6) ng/L,(307.7 ± 15.1 ) ng/L,(60.2 ± 9.3)ng/L,P <0.05].The serum TNF-α,CRP,IL-1,IL-6,IL-10 decreased after 26 and 50 h treatment compared with those before treatment in control group [after 26 h treatment:(262.7 ± 29.4) ng/L,(86.4 ±23.7) mg/L,(29.6 ± 3.1) ng/L,( 175.0 ± 10.6) ng/L,(42.7 ± 5.4) ng/L; after 50 h treatment:(219.3 ±25.6 ) ng/L,(75.6 ± 24.0) mg/L,(23.5 ± 2.8 ) ng/L,(99.0 ± 8.2 ) ng/L,(29.3 ± 4.8 ) ng/L; before treatment:(352.5 ± 40.4) ng/L,( 123.2 ± 35.2) mg/L,(37.5 ± 3.8) ng/L,(308.2 ± 15.3) ng/L,(58.4 ± 8.8) ng/L,P <0.05].There were significant differences in the serum TNF- α,CRP,IL-1,IL-6,IL-10 after 2,26 and 50 h treatment between two groups (P < 0.05).ConclusionAP combined with CVVH can effectively decrease the serum inflammatory mediators in SIRS patients and it's therapeutic effect is superior to mere CVVH.

Objective To study the relationship between intradialytic hypertension (IDH) and inflammatory factors in patients with maintenance hemodialysis.Methods Forty maintenance hemodialysis patients were involved,including 20 patients with IDH (IDH group) and 20 patients without IDH(non-IDH group) during hemodialysis.All of them had no sign of acute infections or other progressive disease.The levels of C-reactive protein (CRP),interleukin (IL)-1 β,IL-6,tumor necrosis factor (TNF)-α were measured by enzyme-linked immunosorbent assay (ELISA) before hemodialysis and 1,2,4 h after hemodialysis.The correlation of hypertension to CRP,IL-1 β,IL-6,TNF-α was analyzed.Results The levels of CRP,IL-1β,IL-6,TNF-α were increased in IDH group with the progress of hemodialysis,but were stable innon-IDH group.Logistic regression analysis indicated that there were positive correlations between CRP (r =0.3652),IL-1β (r =0.3261 ),IL-6(r =03156),TNF- α (r =0.3324) and blood pressure during hemodialysis (P < 0.01 ).Conclusion The levels of CRP,IL-1β,TNF-α,IL-6 are positively related to the IDH in maintenance hemodialysis patients.

BACKGROUND: Whether ceramic alloy can be reused after casting depends on its chemical composition, microstructure, castability, as well as mechanical property. However, the effects of recasting on the composition and microstructure of Ni-Cr ceramic alloys are poorly understood. OBJECTIVE: To investigate the effects of recasting on the composition and microstructure of Ni-Cr ceramic alloys. DESIGN, TIME AND SETTING: In vitro materials observation. The experiment was performed at the Department of Prosthodontics, Affiliated Stomatological Hospital, Fujian Medical University, Instrumentation Analysis and Measurement Center, Fuzhou University, from June to August 2007. MATERIALS: The Ni-Cr ceramic alloys were recast 6 times in argon protection to obtain testing sample with diameter of 30.0 mm, thickness of 1.0 mm and side length of 10.0 mm, thickness of 1.0 mm, namely generation I VI. Simultaneously, the sample produced by initial alloy was called generation 0. METHODS: The original alloy and each generation square plates were analyzed by X-ray fluorescence spectrometry (XRF) for qualitative and semi-quantitative analysis. The microstructure and the phase structure of original alloy and each generation castings which had been polished by buffing machine were observed and analyzed by metallographic microscope and X-ray diffractometer (XRD). MAIN OUTCOME MEASURES: Contents of chemical composition, shrinkage cavity, porosity and inclusion, and distribution of XRD patterns. RESULTS: The results of XRF indicated that there were no significant difference on the main composition (Ni, Cr, Mo, Al, Be) among the Ni-Cr ceramic alloys multiple recast 0-6 times in argon protection and accorded with the demands. The metallographic structure of Ni-Cr ceramic alloys were dendritic eutectic structure, but the size of dendritic crystal grain in the casting coarsening, shrinkage cavity, porosity and inclusion gradually increased, with the increase of multiple recast times. The phase structure of Ni-Cr ceramic alloys were Ni-base solid solution (face-centered cubic lattice structure) and the chromium was widely distributed in the nickel base. There were neither significant difference of phase structure nor the new phase structure appeared among the Ni-Cr ceramic alloys multiple recast 0-6 times. CONCLUSION: There is no significant difference of main composition and phase structure between original alloy and each generation castings in argon protection. With the increase of recast times, the size of dendritic crystal coarsening, shrinkage cavity, porosity and inclusion gradually increased.

Utilizing the traditional Chinese medicine theory, authors of this article discussed the feasibility and research of anti-angiogenesis effect of Chinese medicine on tumor. To develop new drugs inhibiting tumor angiogenesis from the Chinese native herbal medicine has an extremely vital significance in blocking tumor invasion and metastasis, as well as improving the patients' prognosis and their survival rates.

Objective To observe the effect of Pingyi(PY) Mixture combined with chemotherapy to Ⅱ~Ⅲ patients after tumour surgery which can resist transference and recurrence of tumour.Method According to the random principle,120 patients who had suffered the surgery of commonly seen tumour were divided into observed group and matched group.Observed group adopted PY Mixture combined with chemotherapy and matched group adopted simple chemotherapy.After 3 periodic treatments,the effect of the patients after tumour surgery was observed including the ratio of recurrence and transference of observed group within 1 or 2 years,survival ratio,quality of life,body weight,immunological indexes,the toxic and sub-effect,the blood rheology behavior,and compared with simple chemotherapy.Results The ratio of recurrence and transference of observed group within 1 or 2 years was respectively 25.74% and 31.43%,which was obviously lower than that of control group,which was 32.00%,52.00%(P
